Each prayer requires watching a short ad to receive a response. The results vary: praying for fertility might lead to pregnancy; general prayers offer random benefits (money, new friends, etc.); praying for health can cure illnesses—particularly helpful for challenges like "Disco Inferno."
Interestingly, you can also "curse" the BitLife developers instead of praying. This yields unpredictable negative (or sometimes positive!) consequences—losing a friend, contracting an illness, or unexpectedly gaining some cash.
