As one of the few survivors, you're a Meta-Human, tasked with surviving in the vast 256 square kilometers of the continent of Nalcott (Nekot). The terrain is a mix of harsh environments, from frozen tundras and lava-spewing volcanoes to treacherous swamps and deceptive desert oases. Your survival depends on your ability to farm, hunt, or engage in combat.

You'll face off against zombies—corrupted humans—and bizarre dimensional bosses mutated by Stardust. Be cautious, as Stardust can also contaminate your food and water sources, affecting your max HP and sanity.

The crafting system in Once Human is robust, allowing you to gather around 100 gun blueprints across seven categories, along with various accessories, perks, and upgrades. This gives you the freedom to customize your arsenal to suit your playstyle.

Territory building is another significant aspect of the game. With your Territory Core, you can establish a base anywhere you choose. Whether you prefer a simple survival room or an expansive apocalypse mansion complete with a kitchen, patio, and garage, the choice is yours. If you find the location isn't to your liking, you can even relocate your entire base.
