Hand of Wisdom and Action is a Unique Glove that drops from Xesht, We That Are One, the pinnacle boss in the Breach endgame. To encounter Xesht, you need to access the Twisted Domain via the Realmgate on the Atlas using a Breachstone. The Realmgate is conveniently located near the starting point on the Atlas, easily accessible by clicking the home icon to focus the camera.
Alternatively, HoWA can be purchased on the Path of Exile 2 official trading website, though this option is costly as even poorly rolled HoWA can fetch multiple divines.
How to Get a Breachstone in PoE 2
There are two primary methods to acquire a 
Breachstone in PoE 2:
- Combine 300
Breach Splinters: These splinters drop from Breach map nodes on the Atlas. Accumulating 300 of them requires farming numerous Breaches, but once collected, they can be combined into a Breachstone. - Using the Currency Exchange: You can buy Breach Splinters and Breachstones from the in-game Currency Exchange by speaking with Alva and placing an order. The typical cost for a Breachstone is around 50-60
Exalted Orbs, although prices may vary based on market dynamics.
The ideal rolls for a Hand of Wisdom and Action in PoE 2 include at least 4% Attack Speed per 25 Dexterity, 1-12 Lightning Damage per 10 Intelligence, and two rune Socket slots. Achieving these perfect rolls requires using a 
Vaal Orb on a HoWA.
Can You Use Orb of Chance to Get Hand of Wisdom and Action
No, you cannot use an 
Orb of Chance to obtain Hand of Wisdom and Action. This unique item is exclusively dropped by Xesht, We That Are One, and is not part of the standard loot pool, unlike items such as Astramentis, 
Headhunter, or 
Polcirkeln. Therefore, attempting to use an Orb of Chance on White Furtive Wraps will not yield HoWA.
